    If your child becomes ill enough to be removed from school or has more than a minor injury, you will be notified.  It is the parent's responsibility to make arrangements to pick up an ill or injured child as promptly as possible.  When you register your child, you will fill out the "Emergency Contact and Medical Form" online.  This includes phone numbers for you and others that are authorized by you to pick up your child in an emergency.  It is important that you notify the school in person.  Only persons listed on the Emergency Contact and Medical Form" can pick up your child from school.  
     
    To decrease the spread of illness at school, please do not send your child if he/she has a fever, deep cough, rash, diarrhea, or has vomited in the last 24 hours.  Children should be fever-free (without Tylenol or Ibuprofen) for 24 hours before returning to school, regardless of how they feel.  A fever is classified as 100.0 or above.  If your child has been diagnosed with a communicable disease, such as strep throat or pink eye, he/she must have been on antibiotic therapy (and fever-free) for 24 hours before returning to school.  Children diagnosed with chicken pox or head lice must be checked by the nurse before returning to their classrooms.  Also, please notify us of any diagnosed communicable disease, including Covid-19.
     
    When you register your child, please fill out the "Emergency Contact and Medical Form" online completely.  If your child has severe allergies, a chronic disease (including Asthma or Diabetes), or another health concern we need to be aware of,  please notify us as soon as possible.  This allows us to have the necessary accommodations in place for your child when school starts.
     
    Are your child's immunizations up to date?  If in question please speak to your health care provider.  By age 5 children are required by AZ state law to have for school: 5 DPT's, 4 Polio's (IPV), 3 hepatitis, 2 MMR's, and 1 varicella.  At age 11, they are required to have a Tdap and Meningitis immunization.  Children are not allowed to attend school without proper immunizations, per AZ state law.   
     
    Hearing and vision screening will be conducted within the first few months of school on all new-to-New River Elementary students as well as Kinder, First, Third, Fifth, and Special Education students. Parent/guardian will be notified of any atypical results.
     
    Lastly, please encourage your child to wash their hands with soap and water often, especially if he/she has a cold or cough or is around others who do.  Hand washing is the single most effective measure for preventing the spread of infection.
     
    WHAT IF MY CHILD HAS A BATHROOM ACCIDENT WHILE AT SCHOOL?
    This sometimes happens.  Please send a bag, labeled with your child's name, with a change of clothes to keep in the Health Center or a backpack for such occurrences.

     Immunizations DISEASE PREVENTION/IMMUNIZATIONS:

     Written proof of compliance with immunization requirements is a part of the registration process in Arizona Schools. State and Federal Law mandates that school-age children be immunized to prevent certain diseases. If you are unsure of the requirements for immunizations, see your doctor, visit the School Health Center or call the Maricopa County Health Dept at (602) 506-6767 or (602) 263-8856 for locations of clinics and hours of operation. Immunizations are free of charge for children under the age of 18. Exemption forms are available in the health center.
     
     
    CHRONIC HEALTH CONDITIONS:

     If your child has been diagnosed with a chronic health condition, please contact the nurse immediately. He or she will inform you of the 香港六合彩资料 protocols/procedures necessary to ensure a healthy and safe environment for your child during the school day.
     
     
     
    Parent/Emergency Contacts:

    On the first day of school, your student will be coming home with a "pink sheet".  This is our emergency contact form asking you to update your child's records. Please list all contact numbers including names and numbers of those we can call if we are unable to contact you.  Please include any of your child's medical conditions and medications they are taking.
    It is extremely important that I know of any medical conditions and medications.  If this information should change during the school year, contact the Health Office and let me know.

    STUDENT INJURIES AND ILLNESS:

    If a student feels ill during the school day, they should report their complaint to the classroom teacher for a referral to the nurse. Students are not to contact their parents directly via cell phone regarding illness unless prior arrangements have been made with the nurse. Please keep your child home if they have fever, diarrhea, vomiting, deep cough, or a potentially communicable disease. Children with a fever of 100 degrees or more must be fever-free for 24 hours without the use of fever reducing medications before returning to school. Children vomiting or who have diarrhea must be free from sypmtoms for 24 hours with out the use of anti-diarrheal or antimetic medications.  If your child has been diagnosed with contagious illness, please contact the Health Center so parents of other students in the class can be notified. Please call your child's doctor for advice on an ill child. Do not send ill students to school to be evaluated by the nurse.

    MEDICATION:
     
      All medications prescribed by a Doctor now require a Doctor's note to accompany the medication in a pharmacy labled container/box.  If your child needs to take medication during the school day, complete a medication consent form (available in my office) and provide the medication in the original container. We do not supply over-the-counter medication at school.  If your child needs over-the-counter medication; you must bring it in its original container, new, unopened and indicated for the use of children.
     
    Do not     send the medication with your child to school!  No medication is allowed on campus, including cough drops and vitamins, with the exception of epi-pens and inhalers. If your child needs to keep an epi-pen and/or inhaler with them, there is a separate form to complete.  I always recommend that all medications are kept in the Health Office so they are always accessible during an emergency.  The last thing we want to have to do is search the campus looking for a backpack, checking lockers, desks etc. when you really need one of these two medications. 
     
    Medications are not stored in the Health Office over the summer break.  Any medications not picked up within 2 days after school is dismissed for the summer will be discarded.

    PHYSICAL EDUCATION EXCUSES:        

    Students requesting to be excused from PE must bring a note signed by their parent to the Nurse.  The Nurse will then write an "excused from PE" pass for the student to take to PE.  Any request for three or more consecutive PE excuses must be accompanied by a Physician's written order.  PE teachers will be informed of the length of excuse, and any accommodations that need to be made for your child.  If your child has a health condition that causes him/her to miss PE class frequently, a doctor's note may be requested.
